Crewman David Stephenson died after being crushed by equipment on board a diving support vessel. It appears simple safety measures were ignored causing the work accident.

Mr Stephenson 44, of Willow Bridge, Northumberland was airlifted to Aberdeen Royal Infirmary but sadly was pronounced dead.

Stephenson had been working under suspended equipment when is suddenly lowered onto him. The Marine Accident Investigation Branch (MAIB) said the team involved “failed to apply the most basic of safety principles”

An “un-commissioned hydraulic system” was blamed for failing to apply brakes to the equipment. As a result the owner of the vessel has since implemented major changes to prevent an accident like this ever happening again.
